New York families file an IHIP with their district by August 15, or within 14 days of beginning home instruction (8 NYCRR §100.10). Below is a completed example, not a blank form: grades 9–12 are planned in units of study under 8 NYCRR §100.10(e)(3), cumulative across the four years, at the 990-hour annual equivalent. Why the wording is general on purpose
Describe each subject in general terms ("regular composition assignments") rather than specific promises ("one polished essay per month"). General language satisfies the requirement and keeps you flexible if your plans change mid-year.
Every plan below follows that rule. Copy any of them into your own IHIP and adjust the details to your family.
English
Literature study and regular composition assignments at the high school level using our chosen materials.
Social studies (incl. American history, participation in government, economics)
High school social studies coursework progressing through American history, participation in government, and economics across the four years.
Mathematics
Sequential high school mathematics using our chosen curriculum, with regular practice and assessment.
Science
High school science coursework with reading, labs, and investigations.

Regular study in art and/or music including practice, projects, and appreciation.

A high school health course covering nutrition, development, safety, and wellness.

Do I have to use the exact wording in this sample?
No. The wording is intentionally general (for example "regular composition assignments" rather than "one polished essay per month"), which is what district reviewers prefer. Borrow any line and adjust the details to your family.

What subjects must a grades 9–12 IHIP include?
The required subject list is set by grade band. For grades 9–12, plan every required subject under 8 NYCRR §100.10 at roughly 990 hours across the year. The example above shows each one filled in.
Is a parent-written IHIP acceptable to my district?
Yes. The IHIP is the parent plan. The district reviews it for the required subjects and general plans of instruction. It does not need to be notarized or prepared by anyone but you.
